Cornerstone Kids Pre-Registration

If you'd like to pre-register for our Children's Ministry services at either 9am (Pre-K-under) or 10:45am (Nursery-Fifth Grade) please fill out this form to expedite the process. Parent/Guardian names and info are first with an option for kids information after. 

Please fill in all information above for each child regardless if they come or not on the first Sunday.
